5/12/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C entered Jashim home to conduct individual session to focus on anger management. Jashim appeared to be in a positive mood evident his disposition and laughing with QMHP-C. 2. Behavior: Jashim appeared to be cooperative as he was able to discuss with QMHP-C his triggers at times as it relates to what causes him to become angry. 1.Intervention: QMHP-C began session by reviewing from the last session. QMHP-C inquired about any feelings of anger and reviewed any coping skills that he has recently utilized when he felt stressed and frustrated. QMHP-C encouraged Jashim to communicate with a trusted person and/or friend when feeling like his moods are changing and/or angry and aggressive thoughts. QMHP-C engaged Jashim in problem solving and conflict resolutions. 2. Intervention: QMHP-C discussed anger control strategies and reviewed calming skills with Jashim. QMHP-C engaged Jashim in calming and relaxation skills to help him with learning to relax himself when feels frustrated. QMHP-C taught Jashim assertiveness skills and learning to control his feelings and moods. QMHP-C engaged Jashim in an anger worksheet and processed on problem causing behaviors. 1. Response: Jashim was receptive to QMHP-C's suggestions for today's session. Jashim reported that he struggles with aggressive behaviors after feeling that he has not done anything wrong and/or when someone invade his privacy. He stated to QMHP-C that it is his siblings that gets on his nerves and does things to upset him just like his grandmother. 2. Response: Jashim demonstrated positive anger control while completing the handout and was able to communicate his thoughts surrounding her feelings and thoughts.
